The first two were easy... anyone who saw Phantom Menace should know the first, and playing KOTOR will get you the answer to the second.
The rest were EU-specific, not so easy.

can I ask doesn't the bacta substance contain minor elements of kolto substances?

um, I think they were both developed with different substances.
Kolto was for healing in the early ages of star wars while the Bacta were made into tanks that I believe where used to aid the clones in the CIS vs Clone War.
